Step 1 – Mag fill-up ng nomination form na maaring ma-download sa official FB page ng Kasambahay Kasambuhay Pilipinas Awards. Maari din kumuha ng nomination form sa kahit alinmang branch ng Palawan Pawnshop.

Step 2 – I-submit ang nomination form sa opisina ng JCI Senate Philippines Inc. 2nd floor, No. 14 Don Roces Ave., 1103 Quezon City o sa alinmang branch ng Palawan Pawnshop kalakip ang sumusunod:
a. Certificate of Employment
b. Mga Litrato, ID or anumang patunay na 7 taon ka na sa pinapasukan

Deadline for submission of nominations is August 15, 2017
